Information for General Practitioners
GP's can make a referral to me in any of the following ways:
1. Provide the patient with my contact details. The client can then make contact directly for an intake call and initial appointment. Please contact me if you require business cards.
3. Fax a completed Mental Health Care Plan or referral letter for the patient being referred to 02 6282 9002.
4. Contact me directly via telephone to discuss your patient on 0402 676 369.
Information for Clients
A referral is not required to access psychological support. A Medicare rebate is available however, for eligible clients with a Mental Health Care Plan (MHCP) from their GP.
From 1 July 2024, this rebate is $141.84, reducing the cost of therapeutic services.
Self-referral
Clients who do not wish to access the Medicare rebate scheme may self-refer by contacting me directly.
1. Contact me directly on 0402 676 369 to make an appointment. If the phone is unattended, leave a message with contact details and I will return your call within 24 hours.
Social Media
Consistent with APS guidelines, Mike does not use social media forums, such as Facebook or Linkedin, to contact or maintain contact with clients and will respectfully decline any invitations to connect.
